CenCal Health� Procedures Requiring a TAR

The purpose of this page is to help you easily determine whether a procedure does or does not require a Treatment Authorization Request ("TAR"). If you have the code, enter the number of the code in the "Code" field. The "%" is a wild card character, so entering "Z%" will return all procdures requiring a TAR the begin with the letter Z.

If you do not have the CPT or HCPCS codes, enter a key word in the "Key Word" field, e.g. "hysterectomy"and a list of procedure descriptions and codes that require a TAR will be displayed. Choose the description and codes that best fit the procedure or service that you plan to provide and the codes that require a TAR be submitted to our Utilization Management Coordinators for prior authorization will be displayed.

Note that in some cases, only 1 TAR needs to be submitted for multiple procedure codes and in other cases, a TAR will be needed for each individual code.
