Enrolling in CenCal CareConnect (HMO D-SNP) is easy. This Medicare plan is designed for individuals who are eligible for both Medicare and Medi-Cal. This plan combines Medicare and Medi-Cal benefits in one plan.
The CenCal CareConnect plan starts January 1, 2026. Enrollment opens October 15, 2025.
