Sponsorships

CenCal Health proudly partners with organizations to support events—such as health fairs and annual fundraisers—that promote the health and well-being of our members and communities. We advance these efforts by sponsoring nonprofits that align with our mission and vision. To be considered, events or organizations must address at least one of the focus areas outlined in our Sponsorship Request Form.

Please complete the form linked below and email it to community@cencalhealth.org to request sponsorship, preferably at least three months before the event.

Attach the following documents to the completed form, and email all of them together in a single PDF:

• Letter of request on the organization’s letterhead
• W-9 form from the current calendar year
• Breakdown of sponsorship levels and benefits
• List of board members and/or event committee members
• List of confirmed and potential event co-sponsors

We appreciate every submission and carefully review each one. While we cannot guarantee approval, requests that align with CenCal Health’s mission, vision, and focus areas are prioritized. After a request is submitted, a follow-up will be provided with information about the next steps.
