How to Apply
Enrolling in CenCal CareConnect (HMO D-SNP) is easy. This Medicare plan is designed for those who are eligible for both Medicare and Medi-Cal. This plan combines Medicare and Medi-Cal benefits into a single plan.
Enrollment for 2027 opens October 15, 2026.

Between Oct. 1 and Dec. 31, 2026, we are holding seminars. The seminars are open to all our Medi-Cal members:
- With Medicare or
- Who are turning 65 in the next year
Come in person to learn more about CenCal CareConnect. We can take your enrollment at these events. Seating at each seminar is limited, so please call us for a reservation at 1-877-227-3051.
