As a valued CenCal CareConnect D-SNP member, you now have access to our comprehensive Medication Therapy Management (MTM) program at no additional cost. MTM is part of your Medicare Part D benefits and is required by the Centers for Medicare & Medicaid Services (CMS) for members who meet certain conditions. Our clinical pharmacists are here to help you get the most benefit from your medications while avoiding potential problems.

What is Medication Therapy Management?

Medication Therapy Management (MTM) is a Medicare Part D program that helps you get the best results from your medicines. It is offered at no cost to you if you qualify.

Our specially trained pharmacists work with you and your doctors to:

• Make sure your medicines are safe and working well.
• Help prevent harmful side effects or drug interactions.
• Find ways to save you money on your medicines.
