2026 Medi-Cal Outreach and Navigation Program
Program Overview
The Medi-Cal Outreach and Navigation Program promotes continuous Medi-Cal coverage to ensure members maintain access to essential health services. The program funds trusted community partners to deliver culturally and linguistically responsive outreach and navigation support to Medi-Cal members in Santa Barbara and San Luis Obispo counties.
Projects must directly support the Medi-Cal Outreach and Navigation Program goals:
- Increase awareness among Medi-Cal members about renewal requirements to maintain coverage.
- Reduce disenrollment from Medi-Cal due to administrative/procedural reasons through targeted outreach and navigation assistance.
- Prevent disruption in care by assisting Medi-Cal members with re-enrollment during the 90-day cure period.
Technical Assistance Session
CenCal Health will host a Technical Assistance (TA) session on Monday, June 1, 2026, from 11 a.m. to 12 p.m. This session is intended to support prospective applicants in preparing and submitting their applications, and we encourage all interested organizations to attend.
During the session, CenCal Health staff will provide an overview of the Medi-Cal Outreach and Navigation Grant Program, review key application requirements, and go over how to access the grant application on CenCal Health's grant portal. Participants will also have the opportunity to ask questions.

Applications must be submitted electronically through CenCal Health's grant portal no later than Monday, June 15 at 5 p.m..
